I/We understand that participation in classes and activities can contain possible risk of personal injury. I understand that staff will take all measures of precaution, though accidents or injury may occur. By agreeing to and signing this release form, I (the parent/guardian/dancer) acknowledge that I am not eligible for dance insurance under Ambiance Dance in this preliminary period and that Ambiance Dance is released from any injury that may occur within my trial period.
I agree to release Ambiance Dance (including staff members, dancers & facilities) from any cause of action, claims or demands now or in the near future in relation to my trial period and will not hold Ambiance Dance liable for personal injury or any personal property loss or damage which may occur on the premises of Ambiance Dance. Furthermore, I agree to obey the class and company rules and take full responsibility for my/our behaviour and any damage that may be caused to others or the facilities utilised within my time at Ambiance Dance.
I understand that Ambiance Dance is a licensed and insured organisation with qualified staff and in the event that there is an observation of any misconduct or conditions that are deemed unsafe, before, during or after class, I agree to report it to the Director, Instructor or other staff member/s immediately. Should injury occur, I give permission for first aid trained staff to attend to myself/my child.
